Physically or mentally handicapped children may be eligible for continuous coverage after 18 if application is made within 31days of the child’s 19 birthday. Students who becomedisabled after 19 will be eligible for continuous coverage if the student is incapable of attending school due to a mental or physical handicap.

Eligibility

You must have completed 6 months of service and be at least 21 years of age.

Enrollments

Quarterly on January 1, April 1, July 1, and October 1 after eligibility requirements are met.

Employee Contributions

Up to $16,500 (2009); with a catch-up limit of $5,500 (2009).

Employer Contributions

Dollar-for-dollar up to 3% of annual compensation; plus 50% on the next 2% of your compensation.

Vesting

Safe Harbor contributions and earnings 100% immediate. Employer’s profit sharing contributions and earning will follow vesting schedule.

Loans

Not permitted by law.

Distributions

May only occur in the event of: retirement, death, disability, termination of employment, hardships
